For your appointment

  • Insurance - Make sure that we have the most current insurance information prior to your appointment.  If you have been involved in a motor vehicle accident, work injury or applying for disability please inform the staff prior to your visit.  If your insurance requires a referral please contact your primary care physician.
  • Medication List - Please bring a full list of your medications that you take everyday or "as needed basis" and over the counter medication. They may need to be stopped prior to an injection because they may interact with medications that may be prescribed to you by our office.
  • Medical Records - Please bring a copy with you to our office any labs, radiology studies or other physician notes.
  • Narcotic Policy - most often we do not prescribe medications at the first visit.  You will be required to review and sign our narcotic contract.  No narcotics will be prescribed until all previous records, drug scans are obtained.

If conservative treatments do not provide adequate relief, we can offer an array of treatment options.  All procedures are done in a sterile operating room setting, under x-ray guidance and may be completed with a mild sedative.  All of our services are provided in a relaxed, comfortable environment, with sensitivity to your needs.

Our Multi-disciplinary approach many include the following:

  • Assisting in scheduling appointments
  • Ordering of any radiology studies required
  • Physical Therapy
  • Interventional Pain Procedures ( such as epidurals, nerve blocks, facet blocks, etc)
  • Medication Management options
